The Family Support Program provides financial assistance for products, resources or service Items related to the care of a child with special health needs.
Funding for Special Needs
Children with special needs often require unique equipment, supplies and activities that are crucial to their health and/or enhance their quality of life. Often these essentials are not covered by insurance and may be impossible to acquire, or may critically impact families' financial well-being. Funding special needs items is a challenge, but there are sources that can help.
Requests for funds through OCCYSHN can be submitted through a representative of one of the OCCYSHN Programs (CaCoon, Community Connections Network, Family Involvement Network or other OCCYSHN grant activities), or directly to the Family Support Program by a health, education or agency advocate.

General ($750 FSP funds per child, per year)
* Children birth to 21 years of age.
* Families must live in Oregon or pay Oregon income tax.
Medical
FSP is for children who have long-term developmental, physical or medical conditions. All three of the following elements must exist for a child to be eligible:
1. The special health need is complex enough to:
a. Restrict growth and development, or
b. Restrict the ability to engage in usual activities, or
c. Create a functional limitation.
2. The condition requires specialty medical care or more medical services than a typically developing child of the same age.
3. The condition has lasted, or is likely to persist, at least 12 months.
Financial
* Due to limited funding, priority is given to those families who have a true demonstrated need.
* Family financial need will be determined by the individual facilitating the request on behalf of the family. A statement of financial need is required.
* FSP dollars are the payment of last resort.
* Annual cap of $750.00 per child, per year.

* Adaptive equipment
* Assistive technology and educational devices
* Disposable medical supplies
* Medical equipment
* Oral appliances
* Therapeutic activities
* Equipment rentals
* Specialized nutrition
* Nutrition and feeding supplies
* Interpreter services
* Lodging and transportation (reimbursable expenses)
* Orthotics
* Crisis Fund
Zetosch Funds ($1,000 -$1,500 per child, per year)
* A private endowment established to support the purchase of adaptive equipment and assistive technology to enhance educational opportunities for children with special health needs.
* Families must be Oregon residents or pay Oregon income tax and have demonstrated financial need.
* Funds must be used for equipment related to educational needs. Covered equipment include: computers, printers, software, communication devices, hearing aids, eyeglasses, van lifts, tie downs, wheelchairs, ramps, and other mobility items.

Street Rods ($1,000 per child, per year)
* Private funds for residents of Southern Oregon counties (Coos, Curry, Jackson, Josephine,
Klamath, Lake).
* Funds can be used to purchase services, equipment, supplies, adaptive recreation activities, transportation and lodging. Prior authorization of funding is required.
